Repair Pickup Request Form
During the academic year, Ellis Music representatives travel to schools throughout Vermont and New Hampshire on a regular schedule. The form below can be used to request that an instrument rental or a privately owned instrument be picked up for repair. Once picked up, the instrument would be taken back to Ellis Music, where a technician will assess what needs to be done. There is no charge for this estimate. You will be contacted for approval before any work is done to the instrument.
For urgent requests, it is better to call the store at 802-234-6400 during business hours than to use this form.
